Case Management Services
Inpatient Case Management
When you’re admitted to Mount Nittany Medical Center, this team coordinates skilled nursing, acute rehabilitation, home health, hospice, and much more. We also verify that insurance approvals are in place.
Emergency Department Case Management
Available 24/7, this team focuses on everything from mental health assessments, to discharge planning, and community referrals. We are here for you for support during emergency situations.
Ambulatory Case Management
Available at our outpatient primary care locations, case managers help with concerns that can impact your overall well-being, like food insecurity or financial hardship. The case manager will work collaboratively to assist with identified needs.
