Pennsylvania Registered Nurse license or Pennsylvania Practical Nurse License required. CPR certification required. Minimum 3 years experience in a clinical nursing role. Knowledge of nursing principles and skills needed to provide quality patient care and treatment including examination, diagnostic and treatment procedures, medications effects on patients and patient education principles. Knowledge of common safety hazards and precautions. Knowledge of physician office procedures, medical terminology, ICD-9 and CPT coding guidelines. Ability to communicate clearly and effectively and read, understand and follow written and verbal instructions of the physician. Must be able to react calmly and effectively during emergency situations. Must have the ability to identify and analyze problems, prioritize, make confident quick decisions and recommend effective solutions. Ability to administer a variety of medications and treatments and utilize various types of equipment for examination and treatment procedures.
This exciting, new opportunity is now available in several Mount Nittany Physician Group practices. This position will be responsible for providing assistance to the care team by proactively managing and coordinating care to patients and by assisting all patients through the healthcare system by acting as patient advocate and navigator. Other responsibilities include: Coordinates continuity of patient care with external healthcare organizations and facilities, including processing hospital admission, discharge and referrals from the primary care provider to a specialty care provider. Coordinates continuity of patient care with patients and families following hospital admission, discharge, and ER visits. Facilitates patient medication management based upon standing orders and protocols. Performs patient assessment of medical condition required and report to physician or mid-level provider. Screens and prioritizes patient telephone inquiries concerning their medical care, prescription refills, insurance authorizations and referrals and records information in patient chart. Provides appropriate education and instructions to the patient and family regarding medication and treatment protocol. Performs and documents various tests and procedures as directed by the physician. Documents patient information, test and examination results and care provided in the patient’s electronic health record. Contacts patients to provide test results and care instructions according to physician instructions. Using the Electronic Health Record, documents and bills for services rendered. Obtains proper insurance company referral or authorization for required visits or testing.
