Minimum Qualifications
Minimum three years experience required, preferably in an ambulatory setting. Bachelor of Social Work and Master of Social Work from an accredited school required. Experience in collaborating with community based services, pharmacies, durable medical equipment suppliers and home health care required. Demonstrated ability to communicate effectively and express ideas clearly; actively listens. Demonstrated ability to identify and solve problems.
Primary Duties
The primary care practices of Mount Nittany Physician Group is currently recruiting for a seasoned Social Worker (MSW) to provide case management services and assist patients in navigating social service systems.
